Show your professionalism and
ethical standards. Downloadable registration documents are found
below; print off, complete and then submit to the mailing address
included on the form. If you have any questions, please e-mail
us. Practitioners will be notified within 30 days of application;
educators and facilities will be notified within 60 days of application.
SUPPORTING DOCUMENTATION:
- Each applicant must include
proof of activity or existence, including a valid certification,
a business license or the first page of the articles of incorporation,
for example.
- Each applicant must include
a written description, proof or evidence as to how the applicant
will uphold each standard within his, her or its domain. This
can be done by providing one's policies or procedures manual,
business plan or explanation of how business is conducted on
a day-to-day basis. Be brief and clear in your report submission,
and state only what is necessary to demonstrate standard adherence.
A summary should be included as to how the program specifically
supports the standards' essential elements. Reports are to be
submitted annually, with any changes to policies or procedures
distinguished in a different color or "highlighted"
for faster and comparative review by the FSC.
- Applicants must be aware of
any changes to FSC standards and the latest web site uploads,
as "flagged" on this site in the News section.
- Educators must include relevant
learning materials, exams and testing procedures, assessment
instruments (i.e., how it is determined that practitioners have
sufficient knowledge, understanding, and skills), and any resumes
or background information of teaching and assessing personnel
to demonstrate staff competency.
- Practitioners must be active
for at least six months before applying; facilities and educators
must have operated for at least one year before applying.
NOTE: An FSC review board member may contact an applicant
in order to obtain further information or clarification on a
submission. This is not to be interpreted as notification about
the status of an application. Notification will be provided by
the FSC by way of written letter (and a certificate of accreditation
issued to facilities and educators).
